Effective January 1, 2006 the Trustees approved implementing three changes to the current plan benefits:

  1. The plan has moved the coverage of mamograms into the "wellness benefit".  In doing so, the calendar year deductible will no longer apply to mammograms.  In addtion, the $20.00 co-pay for wellness care is removed and the annual maximumhas been increased to $500.00
  2. The annual $1,000.00 maximum for durable medical equipment has been changed to a $1,000.00 calendar year maximum on each item when determined to be medically necessary by the plan.  
  3. The plan has defined a specific benefit for Cardiac Rehabilitation.  Cardiac Rehabilitation has a maximum benefit of $5,000.00 per treatment plan and must meet these specific quidelines;
    -Cardiac rehabilitation therapy must begin within 8 weeks of the event, i.e. hear attac, surgery, etc
    -A format treatment plan must be submitted by the physician to the claim office
    -The therapy must be medically necessary
    -The therapy must be completed within three months
  4. A prenatal program.  This program is for all pregnancies and not limited to "high risk". 
    The "High Risk Pregnancy Benefit" as listed on page 22 of your current Summary Plan Description is no longer in effect.  Click here to find information regarding the benefits of the prenatal program and to see a brochure.
