Effective March 1, 2005 the Board of Trustees of SEIU Local 2000 Health and Welfare Fund approved an amendment to your plan of benefits providing:

A death benefit for each active participant of $5,000.00 and accidental death benefit for each active participant of $10,000.00, there are no benefits payable upon the death of a dependent.

A weekly disability income benefit of $75.00 beginning on the 8th day of disability for a total disability period of thirteen weeks. This benefits will begin on the 8th day of an illness or injury for which you are receiving physician's treatment and are unable to return to work. (This does not include an illness or injury that will be covered by worker's compensation regardless as to whether or not a claim is filed.) This may or may not include an inpatient hospitalization. Physician's treatment does include services form People's Clinic. When you are absent from work as a result of an illness or injury, you will need to contact the fund office. We will forward to you a form that will be completed by both you and your physician. Once the completed form is received in our office, your benefits will begin. Benefits will continue until such time as your physician releases you to return to work or until the expiration of the thirteen week disability period. From this weekly benefit we will deduct Social Security and Medicare taxes.
