The board of Trustees are please to provide employees covered by collective bargaining agreements between employers and SEIU Local 2000, with a Pension Plan. The following companies belong to the Pension Fund: We will be happy to assist you in every way to make certain that you receive promptly the benefits to which you are entitle. If at any time you need information or assistance, please write or call the Plan Office.
For a detailed description of all of your retirement benefits, consult the Summary Plan Descriptions (SPDs) for the Pension Fund. You can receive distribution of your account upon your Early, Normal or Disability Retirement Date, if you terminated your employment by an Employer that contributes to the Plan. You have to designate a beneficiary and she or he will receive benefits payable from the Plan as a result of your death. If you are married the beneficiary shall be your spouse. If you do not have a spouse at the time of your death, or if no other person designated as beneficiary survives you, the beneficiary shall be your state or whoever you designated. If you are married and designate other person than your spouse, we have to have your spouse's consent. Please fill a beneficiary and a spouse's concent form
